Membership Application

The  CFUW Oakville Membership Application form for the year 2024-2025 is available now.  (Download link is below)

This is a fillable form so you may type your entries, save and email the completed form as an attachment to membership@cfuwoakville.ca .  Payment may be made via e-transfer to CFUW Oakville at fees@cfuwoakville.ca 

When you are renewing online please use the two different addresses: mail your application form to membership@cfuwoakville.ca and send your bank transfer to CFUW Oakville at fees@cfuwoakville.ca. This is the only address that is able to receive your payment.

The treasurer does not necessarily see the application form so it will be very helpful if you include detailed information about your payment in the message space provided with the bank transfer:

  • The member’s name if the name on the bank account is different from your name or if we have two members with the same surname
  • The amount of the membership fee you are paying – e.g., Regular membership $129.
  • Details of amounts included above the membership fee – e.g., additional scholarship donation $50; name tag, $12; Directory delivery, $5.

If you prefer to pay by cheque, you may print the form, complete it and mail it with your cheque made payable to CFUW Oakville to the address shown on the form:  CFUW – Oakville, P.O. Box 30048, 478 Dundas St. W. Oakville, ON, L6H 6Y3.
